State Tax Commission Fails to Detect Data Breach for 18 Months – Taxpayer Names and Social Security Numbers at Risk


A US tax agency has put taxpayer information at risk in the wake of a long-running data breach that lasted 18 months.

The Oklahoma Tax Commission (OTC) said the violations occurred between July 2024 and December 2025, according to the new filing. reservation and the Maine Attorney General’s Office.

The agency didn’t notice the incident until December, when it discovered there was no access to some W-2 and 1099 files on its online taxpayer website.

The filing shows that names and social security numbers were stolen in the breach, though OTC did not say how many people were affected.

The agency says it has an “ongoing commitment to the privacy of its users” and will review its existing security standards.

“Privacy and security are among OTC’s top priorities. OTC’s response included completing the investigation, cooperating with the Internal Revenue Service (“IRS”) to continue its investigation to monitor fraudulent tax filings, and conducting a detailed review of the files obtained to identify the individuals involved.”

OTC also said it will offer access to credit checks and personal protection services for one year through Cyberscout, a TransUnion company.
